It shouldn't. We use sodium ascorbyl phosphate rather than L-ascorbic acid specifically because it's tolerated by sensitive skin. If you're prone to reactions, patch test on your inner arm for two days first — that's good practice with any new active.
With niacinamide, yes — the old claim that they cancel each other out has been widely debunked. With retinol, use vitamin C in the morning and retinol at night rather than layering them.
Because it hasn't oxidised. A fresh, stable vitamin C serum should be nearly colourless to pale straw. If yours has gone orange, it's past its useful life.
